New parishioners are always welcome! Please print out the form below and mail your registration to the church or drop it by the church office during regular business hours. Also, please notify us if you move out of the parish or when you have a change of address. Thank you.

PLEASE NOTE: Page one is to register for all classes at all grade levels except sacramental preparation classes (1st Reconciliation, 1st Communion, & Confirmation). Please fill out a separate form form if your child is in any of our sacramental preparation classes. See below for instructions:
Page two is the Reconciliation/Eucharist registration form.
Page three is the Confirmation registration form.
Page four is the media & medical release form. You may use one form per family on this. Please enter the first names of your children on the form. Please make sure to read the information at the top of the page. Page four should be printed along with whatever registration form you’re filling out.
